It's all clean and the lever is set so it does not free flow with the diaphram setting on it. If you take several real quick breaths the diaphram seems to drop down and stick in the down position and the regulator flows wide open till you either pick up the diaphram or shut off the air. Without the diaphram on it you can push the lever down all the way and it springs right back up? Has anyone ever had this problem? If so how did you fix it ?

Sounds like the venturi effect is causing a vacumn and sucking in the diaphram. I have had this happen when making ajustments before clamping the two haves together and putting the hoses on. Was this happening while you were sucking on the box outlet or through the inhalation hose. The hose produces enough resistance to flow to limit the venturi effect vacumn from pulling in the diaphram. If you didn't have the inhalation hose on put it on and try again.
