John,
I am going to cheat here, I know of one that you ask about,
Scott Carpenter - Mercury/Sealab II
as for the others, you forgot about the Aquarius program off Key Largo, I can name at least 4 more that were on NEEMO 6 alone!
John Herrington-STS-113 Endeavour Douglas H. Wheelock-STS-120 Nicholas J.M. Patrick-STS-116 Tara Ruttley-International Space Station (ISS)
There have been a bunch more than that know due to Aquarius. Lots of cross training between NOAA and NASA now, there is even a program currently going on this week I think.

Los Angeles County Parks and Recreation created the first recreational scuba diving certification course in this country in 1954.
However, YMCA was the first organization to certify SCUBA Instructors and divers based on defined national standards in 1959.

Hey Melvin, have you ever seen a scubapro regulator with the words scubapro lightly on the front cover,like they were sanded off.I have the one with the raised letters also. Scu ba pro, Jerry Lang Saw you in Cozumel couple of years ago!!!!!!!!

I'm not too sure about Great Britain, but the Aqua Lung was marketed in Canada in 47 or 48, and it was marketed in the United States in 1949 by Spaco.
Anyhow, What month & year was the first US Divers catalog produced?
